10/27/2023 EnchantaVaughn Approved Behavior: As the session began, client took a few minutes to gather himself before going to talk to QMHP. Client was able to redirect his behaviors and join the session with a more positive attitude. Client was excited to see his counselor and continued working quietly. Client talked about his feelings without displaying hyperactive or impulsive behaviors and was supported for his maturity and independence at this time. Client was monitored and supported with processing his feelings and thoughts about everything he was dealing with at home. Behavior: Client was able to transition back to therapeutic activity in a good mood. Client displayed great participation skills in all his home activities. Client was engaged and focused, indicating he wanted to earn an incentive. Client was observed being courteous and getting along well with his family. Client was acceptive of feedback provided by QMHP. Client appears to be content and stable mood, as observed by positive and calm behaviors. Client displayed age appropriate skills and impulsive, hyperactive behaviors were not present at this time. Intervention: QMHP will meet with client daily to discuss progress and expectations that are working and not working in the home weekly. QMHP inquired of clientsâ current mood. QMHP utilized active listening skills as client spoke of his current mood. QMHP utilized active listening as information was conveyed. QMHP will provide client with support and encouragement. QMHP will continue to build rapport. QMHP communicated with client on displaying appropriate control. Intervention: QMHP reflected with client on his day, reviewing both good and bad experiences. QMHP provided a summary of the session, to reassure client had clear knowledge of the purpose of session. QMHP directed client to verbally express positive affirmations. QMHP will explore positive self-talk, use instruction modeling, and increase social communication skills with positive reinforcement role play activity. QMHP will educate client on behavior modification to help client identify what behavior he needs to decrease and avoid daily. Response: Client was able to reevaluate himself and display a more positive attitude toward peers and authority. Client was observed having positive interactions with everyone in the home. Client demonstrated responsibility and leadership by helping his mom around the house. Client implemented his plan for restoring relationships, and this was reviewed. Client demonstrated expectations and responded well to feedback. Client continued to interact in a positive manner with his mother and counselor. Response: Client took his mind from thinking and doing to a place of concentration, which allowed him to focus on one thing at a time. Client nodded in agreement. Client was observed displaying age appropriate behaviors and was productive and studious throughout the day. Client demonstrated clear understanding of objectives. Client expressed positive self-talk, modeled positive behavior, and good communication skills during role play activity.
